About the role
We are looking for a compassionate and structured Case Worker to support clients in navigating social services, crisis intervention, and community resources. You will assess individual needs, develop goal plans, and advocate on behalf of clients to improve their overall well-being.
What you'll do
- Conduct intake assessments to evaluate client needs, background, and eligibility for assistance programs.
- Develop customized case plans, setting attainable goals and timelines with clients.
- Connect clients to essential resources, including housing, healthcare, employment, and mental health support.
- Maintain detailed, accurate, and confidential client documentation and case notes.
What we're looking for
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field.
- Strong active listening, empathy, crisis management, and conflict resolution skills.
- Familiarity with local community resources and social service compliance regulations.
